Job description

Program Overview Support dual‐eligible Medicare and Medicaid members in Kankakee, Iroquois, Livingston, Ford, Champaign and surrounding counties. Provide case management to improve health outcomes. Position Summary / Mission Case Management Coordinator collaborates within the case management team and streamlines care for members through appointment scheduling, benefit education and care plan implementation. Key Responsibilities Evaluate members' eligibility and need using care tools and recommend case resolutions. Identify high‐risk factors and refer to clinical case management or crisis intervention. Coordinate and monitor assigned care plan activities. Consult with case managers, supervisors, medical directors and other programs to overcome barriers and present cases at conferences. Escalate quality‐of‐care issues and use negotiation skills to secure needed services. Use motivational interviewing to promote member engagement and lifestyle change. Document care activities in compliance with regulatory and company policies. Participate in team meetings and provide peer mentoring or training. Support members in engaging with providers for decision‐making. Required Qualifications Preferred experience in case management and discharge planning. At least two years in behavioral health, social services or equivalent. Preferred managed‐care experience. Strong analytical and problem‐solving skills. Excellent communication, organizational and interpersonal skills. Ability to work independently and use virtual collaboration tools. Willingness to travel within the geographic area. Proficiency with MS Word, Excel, Outlook, PowerPoint and proprietary systems. Education Bachelor's degree or non‐licensed master's‐level clinician in behavioral health or human services (psychology, social work, marriage & family therapy, counseling). Anticipated Weekly Hours 40 hours per week. Pay Range Base hourly rate: $21.10 - $44.99. Additional incentive programs and benefits may apply.
