POSITION PURPOSE:
Plans and administers medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint and bone diseases, to restore function, relieve pain, and prevent disability by properly and effectively administering evaluation and treatment programs.
ESSENTIAL FUNCTIONS:
  • Evaluates, plans, implements, and supervises treatment programs as orders and reviewed by physician in order to upgrade patient's physical capabilities.
  • Consults with other specialists and reports patient's progress to physician, home care agencies, and others as needed.
  • Reviews patient's functional needs and adjusts care plan as indicated to ensure optimal patient outcome.
  • Educates patients, families, and caregivers in treatment and expected outcomes. Incorporates participation by caregivers whenever appropriate.
  • Provides supervision to PTA and delegates appropriate clinical activities based on level of staff and regulatory requirements.
  • Directs and communicates effectively with other professionals and support staff in order to achieve positive patient outcomes.
  • Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains all required prescriptions and signatures.
  • Acts as a resource to promote physical therapy within the community.
  • Educates other health care providers, employers and organizations in physical therapy areas to include but not limited to: ROM, transfers, body mechanics, ergonometric principles, use of assistive devices and gait patterns.
  • Provides In-Home services to patients as ordered.
  • Participates in the performance improvement process by identifying opportunities for improvement and taking action to improve patient care services.
  • Assists clinical instruction for PT and PTA students through university affiliations. Supervises job shadow and high school student interns.
  • Performs other related duties as assigned.
  • Specific to Home Care:
  • Generates weekly schedules to keep office informed of caseload status.
  • Performs admission visits for therapy only patients.

SPECIALIZED KNOW-HOW & REQUIREMENTS:
  • Bachelor's Degree in Physical Therapy required.
  • Illinois Licensure in Physical Therapy required within 6 months of hire.
  • Must obtain CPR certification within 30 days of hire and maintain throughout employment at CGH.
  • Valid Illinois Vehicle Operator's License required.
