Job description

WAYS Home Care & Health Agency is seeking a compassionate, organized, and motivated CFS Coordinator to support adults with developmental disabilities in achieving greater independence while remaining safely supported in their homes and communities.

In this role, you'll build meaningful relationships with individuals and their families, coordinate services, develop person-centered support plans, and connect clients with the resources they need to thrive. What You'll Do

  • Coordinate Coordinated Family Supports (CFS) services for Regional Center clients.
  • Develop and monitor individualized, person-centered support plans.
  • Collaborate with families, Regional Center Service Coordinators, providers, and community partners.
  • Connect clients with housing, transportation, medical, educational, vocational, and community resources.
  • Conduct regular check-ins to ensure services are meeting each client's goals and needs.
  • Maintain accurate documentation and case notes in accordance with Regional Center and agency requirements.
  • Assist individuals in developing independent living skills and increasing community engagement.
  • Advocate for clients while promoting dignity, choice, and self-determination.
  • Monitor service quality and address concerns in a timely and professional manner.

Qualifications

  • Experience working with individuals with developmental disabilities or within the Regional Center system preferred.
  • Knowledge of Coordinated Family Supports (CFS), Independent Living Services (ILS), Supported Living Services (SLS), or related programs is a plus.
  • Excellent communication, organization, and problem-solving skills.
  • Ability to manage multiple cases while maintaining attention to detail.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Valid California Driver's License, reliable transportation, and current auto insurance.
