TITLE: Certified Care Attendant
REPORTS TO: RN Case Manager
Job Summary:The Home Health Aide/ Universal Worker will provide personal care services as described in the client's plan of care, in our 24 hour customized living facility. This position is responsible for providing personal care and designated health-related services designed to maintain the client's physical and emotional wellbeing.
Qualifications: Must be active on the Minnesota Nursing Assistant Registry. Must be able to read, write and speak English. Must be able to follow directions and perform assigned tasks. Must be able to relate work with geriatric clients and understand their special needs. Complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.
