Job description

Overview
At Catholic Health Home Care, we provide strong, talented leaders and foster growth opportunities for our employees. We are one of the largest healthcare systems on Long Island, offering daytime schedules, travel reimbursement, competitive health benefits, and continuing education benefits.
We are seeking dedicated Physical Therapists to care for patients in communities across Long Island. As part of our team, you will work closely with a supportive patient care team and provide care to the highest standards.
Job Details
Our Therapists:
  • Review patient charts and medical history
  • Develop personalized treatment plans
  • Teach therapeutic exercises and stretches
  • Use hands-on therapy to promote healing
  • Maintain patient records and adapt treatment plans accordingly
  • Work closely with a supportive patient care team
  • Provide patient and family education for health promotion
  • Provide care to the utmost standard

Qualifications:
  • New York State PT license
  • Minimum of one year of recent hands-on experience as a Physical Therapist
  • Valid New York State Driver's License and reliable transportation.

Benefits:
  • 5x 8-hour shifts (8:30AM to 4:30PM) with rotating weekends and holidays

  • Education reimbursement of $5,250 annually
  • Competitive salary range of USD $90,000.00 to USD $110,000.00 per year
