Starting Pay: $25.49โ€“$26.93 per hour, approximately $53,000โ€“$56,000 annually
Work Arrangement: Hybrid/Remote with required travel
Service Area: Trillium Health Resources catchment area
Openings: Multiple positions available

Help People Flourish in Their Homes and Communities

Healthkeeperz is seeking compassionate, organized, and highly qualified Care Managers to coordinate whole-person care for individuals with behavioral health conditions, intellectual or developmental disabilities, traumatic brain injuries, and other complex needs.

This position combines remote work with required in-person visits to members in their homes, residential settings, care facilities, and other community locations.

Required Credentials

Applicants must meet all applicable credentialing requirements:

  • Bachelorโ€™s degree in health, psychology, sociology, social work, nursing, or another relevant human-services field; or current licensure as a Registered Nurse
  • At least two years of direct experience working with individuals with behavioral health conditions, I/DD, or TBI
  • Care Managers serving members with LTSS needs must have at least two years of experience in LTSS or HCBS coordination, care delivery monitoring, or care management
  • Valid driverโ€™s license, reliable transportation, and ability to travel throughout the assigned service area
What You Will Do
  • Complete comprehensive assessments and develop person-centered care plans
  • Coordinate physical health, behavioral health, pharmacy, LTSS, and community-based services
  • Address social determinants of health and connect members with needed resources
  • Educate members and their representatives about services, rights, benefits, and available providers
  • Collaborate with providers, facilities, Care Management Extenders, and multidisciplinary team members
  • Support members during hospital admissions, discharges, transfers, and other changes in condition
  • Monitor services, maintain timely documentation, report critical incidents, and escalate complex cases
  • Participate in the agencyโ€™s 24-hour care-management coverage rotation
  • Conduct required face-to-face visits in membersโ€™ homes and community settings
What You Bring
  • Experience developing plans of care from comprehensive assessments
  • Knowledge of Medicaid, behavioral health, I/DD, TBI, waiver, LTSS, or HCBS services
  • Proficiency in person-centered planning and motivational interviewing
  • Strong communication, organization, conflict-resolution, and independent decision-making skills
  • Ability to manage deadlines, documentation, and multiple complex cases
  • Proficiency with Microsoft Office and electronic care-management systems
