Job description

Responsibilities:
  1. Meet and greet each new admission and their family.
  2. Ensure that the room is ready for new admissions to include DME, welcome packet and gift.
  3. Upkeep of the welcome packet. Activity calendar, Monthly newsletter, Wifi code, Resident Rights, Smoking Policy, List of facility extensions and contact information, etc (We are in the process of transitioning to a non- smoking facility)
  4. Assist with labeling of clothes. If the family will do laundry, place a sign in the inside of the closet notifying the staff.
  5. Ensure the diet communication slips gets to the kitchen and the resident receives a tray without delay.
  6. Remind families of their Delivery of Service Meetings and Discharge Planning Meeting
  7. When a resident has to transition to the long term part of the facility, the Concierge will show the resident the room, assist with packing and unpacking the residents items, ensuring that the new room is ready.
  8. If the resident has dentures, hearing aids or eyeglasses assist with securing those items.
  9. Prepare the resident for discharge from the facility by packing their clothing, ensuring all clothing is gathered from the laundry.
  10. Assist with grievance resolution
  11. All new admits will be given a coupon for a free haircut or wash and set. The Concierge will ensure the new admit receives their coupon and taken to the beauty shop. The Concierge will ensure those tickets are gotten to the Business Office for reimbursement from the Business Office/petty cash.
  12. Assist admissions with the input of referrals into PCC/CRM
  13. Assist with signing in of new admits and giving tours.
  14. Assist with scanning in of the Admission packet into PCC.
  15. Escort residents on appointments.
