Job Summary Case Manager:
The Case Manager assists the primary therapist in the coordination of all treatment services through the provision of case management for all residents on his/her caseload. The unique job functions outlined are designed with a focus on safety and quality.
Essential Functions:
  • Provides case management services and coordinates all treatment services to assigned residents.
  • Completes and implements all required Case Management documentation, including, but not limited to: Facilitation and documentation of Child & Family Team meetings. Development of Person Centered Plans (PCPs), and other written documentation as required within the time frames established by New Hope Carolinas, contractual and regulatory agencies.
  • Facilitates, in conjunction with the team Psychiatrist, the resident's treatment team and participates in team planning for the resident; implements the Case Management oriented decisions of the treatment team.
  • Maintains regular contact with families, legal guardians, representatives of referral agencies, and courts (as required); provides regular updates on resident progress and ensures that residents, families, legal guardians and representatives of referral agencies are included in all aspects of planning for the residents.
  • Provides resident progress information to funding sources in a manner and timelines required by the funding source.
  • Identifies resident needs and ensures that identified needs are met.
  • Meets weekly during scheduled team meetings with non-LCS staff to discuss resident needs, appointments and progress.
  • Ensures active discharge planning is completed for each assigned resident, by facilitation of applications for discharge or transitional placements and other written documentation as required within the time frames established by New Hope Carolinas, contractual and regulatory agencies.
  • Strengthens family and kinship relationships
  • Facilitate and coordinate visitations

Qualifications:
  • Bachelor's Degree with a minimum of 1 year or more in the Human Services field (QMHP credential).
  • Must be 21 years of age
  • Must pass a background check and drug screen
