Job Title: Case Worker
Location: New York, NY
Duration: Contract
Schedule: Monday-Friday | 7 Hours/Day | 35 Hours/Week
Pay Rate: $35-$38/hour
Position Overview
A leading healthcare organization is seeking an experienced Case Worker to provide social services and case management support. The ideal candidate will assess client needs, determine eligibility for services, develop care plans, and coordinate with interdisciplinary teams to ensure appropriate support and access to resources.
Key Responsibilities
  • Conduct comprehensive assessments to identify clients' social, financial, and healthcare needs
  • Develop, implement, and monitor individualized service and care plans
  • Determine eligibility for public assistance, medical programs, and community resources
  • Provide counseling, guidance, advocacy, and case management services
  • Collaborate with healthcare professionals and interdisciplinary teams to ensure continuity of care
  • Maintain accurate, timely, and compliant documentation of case activities
  • Assist clients in accessing community resources and support services
  • Investigate client situations and recommend appropriate interventions
  • Ensure adherence to organizational policies, procedures, and confidentiality standards
Required Qualifications
  • Bachelor's Degree from an accredited college or university
  • Minimum 3 years of experience in case work or social services
  • Strong communication, organizational, and documentation skills
  • Ability to manage multiple cases in a fast-paced environment
  • Ability to work onsite Monday through Friday
Preferred Qualifications
  • Experience with EPIC and/or Careport systems
  • Prior experience in a hospital or healthcare setting
  • Knowledge of public assistance programs and community resources
