How you make a differenceThe CalAIM Case Worker supports the successful reintegration of CalAIM-eligible individuals by helping coordinate the development and execution of reentry care plans under the direction of the RN Discharge Planner. This role assists with Medi-Cal eligibility screenings and identifies individuals who qualify for CalAIM Justice-Involved services. The position works closely with managed care plans, in-reach providers, and community partners to coordinate timely communication and service delivery. The role helps ensure individuals are connected to needed services and supports as they transition back into the community.Key Responsibilities
  • Coordinate with Medi-Cal eligibility technicians and Health and Human Services staff to support appropriate activation of Justice-Involved aid codes and service timing.
  • Gather clinical and logistical information, monitor release dates, document consents, and support continuity-of-care handoffs.
  • Ensure timely documentation in the EHR and coordinate with in-reach partners and Medi-Cal eligibility staff to support service delivery.
  • Activate Justice-Involved aid codes in the state reentry portal in accordance with required release timelines.
  • Support continuity of care and compliance with CalAIM requirements under the clinical oversight of the RN Discharge Planner.

Education

  • Bachelor's degree in Social Work, Business Administration, or other health-related fields from an accredited college or university is preferred

Experience

  • Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred.
  • Strong communication, documentation, and coordination skills, as well as a working knowledge of Medi-Cal processes and CalAIM policy requirements, are essential.

Licenses/Certifications

  • Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.
