Case Manager - PHP/IOP
Job Description:
Provive Wellness of Brentwood is seeking a Case Manager to support clients in our Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP). This role is central to coordinating each client's care journey, connecting clinical treatment with the practical resources and support needed for lasting recovery.
Responsibilities:
Manage a caseload of PHP/IOP clients, coordinating care across the treatment team. Conduct intake, discharge planning, and level-of-care transitions in collaboration with clinical staff. Connect clients with community resources, housing, employment, medical, and support services as needed. Communicate with families, referral sources, insurance providers, and outside providers to support continuity of care. Document case management activity, treatment plans, and progress notes in accordance with program and regulatory standards. Monitor client attendance and engagement, addressing barriers to participation. Assist with aftercare and relapse prevention planning prior to discharge.
Qualifications:
Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field preferred. Experience in case management within a behavioral health, PHP/IOP, or substance use treatment setting preferred. Strong organizational, communication, and interpersonal skills. Knowledge of community resources and levels of care within behavioral health treatment. Comfortable working with clients experiencing mental health and/or co-occurring substance use disorders.
