Join our Top Places to Work health and human services Agency, which has provided world-class services for the Jewish and non-Jewish communities in Northeast Ohio for 150 years. Immediate opening for a full-time salaried Registered Nurse Case Manager to provide comprehensive, professional, and compassionate nursing care to patients in their homes. The RN Case Manager will assess and treat clients according to individualized plans of care, monitor and document clients' conditions, progress, and outcomes, and provide nursing education to clients and their families.
Requirements
Duties and Responsibilities:
  • Completes and submits required documentation, including OASIS, according to Agency policy and condition of participation within defined time frames.
  • Documents each clinical visit; minimally, each visit should include a physical assessment of the client, vitals, medication review, skilled interventions and teachings, and an assessment of the home environment.
  • Performs ordered nursing procedures and treatments; initiates preventative, therapeutic, and rehabilitative nursing services, in accordance with State law and within the scope of a registered nurse.
  • Observes and reports any signs or symptoms indicative of changes of condition/change in family situation to Clinical Supervisor, physician, and other health care providers; ensures changes are included in the clinical record.
  • Ensures physician orders are followed and updated appropriately.
  • Collaborates and coordinates with other health care providers and third parties involved in the patient's care.
  • Maintains appropriate, professional, and timely communication with client, client representatives, physicians, the Agency and other parties involved in the client's care.
  • Teaches clients, family and caregivers; provides education regarding medications, disease management, home safety, dietary needs, and how to reduce hospitalization risk.
  • Supervises other agency personnel including but not limited to home health aides and licensed practical nurses.
  • Participates in case conferences and Quality Assurance and Performance Improvement Programs.
  • Travels throughout the geographic market to provide nursing services.
  • Provides on-call support, as needed, to ensure 24-hour staff availability.
  • Understands and adheres to Agency policy and procedure, including client confidentiality and HIPAA regulations.
  • Attends in-services as scheduled.
  • Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices.
  • Performs other duties or special projects, as required

Education/Training/Certifications/Licenses:
  • Graduate of an accredited school of professional nursing
  • Current RN License ( State of Ohio)
  • Two years of prior clinical experience
  • One year of in-home or hospice setting
  • Ohio DL and auto insurance
  • BLS/CPR Certification
  • Reliable daily transportation
