Job Summary

Quadrant Health Group is seeking a dependable, organized, and client-centered Case Manager to support care coordination across our behavioral health programs. This role will support clients across Greater Texas Behavioral Clinic, Des Moines Wellness, and Wellness Ranch Kentucky’s Virtual SUD/MH IOP/OP program.


The Case Manager will serve as a key point of contact for clients, providers, treatment teams, and external stakeholders. This position is ideal for someone who is highly organized, documentation-focused, comfortable with frequent communication, and able to balance compassion with strong follow-through in a fast-paced behavioral health environment.


Key Responsibilities

  • Coordinate care across multiple behavioral health programs, ensuring seamless communication, scheduling, and service delivery.
  • Serve as a central point of contact for clients, providers, internal teams, and external stakeholders.
  • Facilitate psychoeducational and support groups for outpatient and virtual programming.
  • Communicate regularly with clients to assess needs, identify barriers, and connect them with appropriate services.
  • Schedule and coordinate provider visits on behalf of clients to support continuity of care.
  • Maintain timely and accurate case management documentation in Kipu, including notes, releases, contacts, and required records.
  • Submit updates and required documentation to probation/parole officers, legal entities, employers, or other stakeholders as needed.
  • Support admissions, ongoing client engagement, discharge planning, and aftercare coordination.
  • Provide clients with appropriate recommendations for continued care, including primary care, psychiatry, outpatient therapy, peer support, and community resources.
  • Track client engagement, attendance, no-shows, authorizations, overall progress, and compliance-related items.
  • Identify risks, barriers, or client concerns and escalate appropriately to clinical or administrative leadership.
  • Maintain professional communication with clients, providers, treatment teams, and outside parties.
  • Support a structured, accountable, and compassionate treatment environment.


Requirements

  • 1–3+ years of experience in behavioral health case management, care coordination, or a related role.
  • Experience working with substance use disorder and/or mental health populations preferred.
  • Group facilitation experience strongly preferred.
  • Familiarity with payer processes, compliance expectations, and community resources.
  • Strong documentation skills and ability to maintain audit-ready records.
  • Experience with Kipu or another electronic medical record system preferred.
  • Strong communication, organization, follow-through, and time management skills.
  • Ability to work in a fast-paced environment while managing multiple priorities.
  • Comfortable communicating with providers, clients, families, legal contacts, employers, and outside stakeholders as needed.
  • Ability to balance structure, accountability, and compassion when working with clients.


Education & Training

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field preferred.
  • Equivalent professional experience may be considered in lieu of degree.
  • Relevant certifications in case management, peer support, recovery support, or care coordination are a plus.


Schedule Requirements

This is a full-time, on-site position based in Boca Raton, FL.


Please note that this position requires availability to work until 9 PM or 10 PM EST on two predetermined days each week. Applicants must be able to accommodate this schedule.


On assigned group facilitation days, the schedule will shift to either 1 PM–9 PM or 2 PM–10 PM, depending on the assigned facility and program. Remaining workdays will generally follow a standard 9 AM–5 PM schedule.



Compensation

This position offers an annual salary range of $40,000–$52,000 (depending on experience)
