Job description

Registered Nurse - Case Manager

Director of Case Management position working full-time day shifts (8:00a–5:00p) with no weekend requirements. Role oversees all daily hospital case management operations in a 516-bed acute care facility. The Director is responsible for integrating patient care coordination and discharge planning processes across departments to ensure continuity of care and optimal resource utilization. This leader supervises approximately 31 FTEs and collaborates with executive and clinical leadership to achieve quality, operational, and financial goals. Position reports to the Division VP of Case Management.

Requirements: Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred. Active RN, LCSW, or LMSW license required. Minimum 3 years of acute care hospital case management experience required, along with at least 2 years of leadership experience. Recent acute care experience required. Strong executive presence, communication, team-building, and change management skills necessary. Demonstrated ability to lead in a large, complex hospital environment preferred.
