The Impact You'll Make

As our Evening Case Manager, you’ll be the steady, empathetic presence for clients during critical evening hours, when needs can be urgent and resources limited. You’ll ensure that each person you work with feels heard, supported, and guided toward the next step in their journey. Your work will directly influence safety, stability, and hope for those we serve.

What You'll Do

  • Provide case management services to clients during evening hours (3:00 PM – 11:00 PM), ensuring timely support and follow-up.
  • Conduct intake assessments, develop individualized service plans, and connect clients to appropriate community resources.
  • Monitor client progress, document interactions in case management software, and maintain accurate, confidential records.
  • Collaborate with shelter staff, social workers, and partner agencies to coordinate care and resolve urgent needs.
  • Respond to crises with calm, solution-focused action, following established safety and escalation protocols.
  • Participate in evening team meetings and contribute to continuous improvement of services.

Requirements

  • 2+ years of experience in case management, social services, or a related field (evening or crisis work experience a plus).

  • Knowledge of local community resources, housing programs, and social service systems.
