Salary: $45,000 - $50,000

RESPONSIBILITIES

Recruitment/Assessment/Care Coordination

  • CMA will perform outreach/recruiting within the community,informing perspective clients ofCM services and scheduling assessments for eligibility.
  • CMA will schedule and completean InitialScreening appointment using ashortscreener toassesseligibility for CM services and educate onPrEP.
  • CMA will gather eligibility documents and upload/update them ineCWas needed.
  • Referrals will be made for CM Intake appointments, other FMCS services or other communityorganizationsto address client needs.
  • HIV testing and Educationwill be provided tostatus neutral clients.
  • ProvidePrEPeducation and referrals.

Office Hours:Monday-Friday,8:30am 5:30pm


Qualifications:Associates/bachelors degree in healthor human services related field preferred. High School diploma or GEDrequired. A minimum of 2 years of experience working with persons living with HIV or athigh riskof HIV acquisition preferred.HIV Testing backgroundpreferred.
