About Horizon Case Management CORP
Horizon Case Management provides collaborative case management and care coordination services for individuals recovering from auto accidents, workplace injuries, and complex medical conditions. Guided by the CMSA standards, we focus on advocacy, communication, and coordination to ensure every client receives safe, effective, and compassionate care.
Our mission is to help clients feel secure, empowered, and supported by a professional healthcare advocate.
Position Summary
The Field Case Manager supports clients through advocacy and care coordination by attending medical appointments, facilitating communication among healthcare providers, and ensuring continuity of care following the plan established by the Director of Nursing.
This role does not involve clinical assessment, planning, or evaluation - it focuses on care coordination, advocacy, and support. The ideal candidate is compassionate, organized, and skilled at building rapport with clients, medical professionals, and insurers.
Key Responsibilities
  • Attend client medical appointments (approximately 80% travel) to advocate for clients and support care coordination.
  • Collaborate with the Director of Nursing and case management team to communicate updates from providers and ensure the plan of care is followed.
  • Serve as a liaison between clients, physicians, therapists, and insurance representatives.
  • Document all appointments, interactions, and updates in the case management software.
  • Assist clients with understanding their treatment plans and improving health literacy.
  • Attend weekly team meetings for support, case review, and collaboration.
  • Check into the office 3-5 times per week depending on proximity to the nearest Horizon office.
  • Maintain professionalism and uphold Horizon's values of advocacy, reliability, and compassion.

Qualifications
Education & Experience:
  • Required: Background in healthcare or social services - including but not limited to EMT, Medical Assistant, LPN, RN, Social Worker (BSW/MSW), Certified Case Manager (CCM), Certified Brain Injury Specialist (CBIS), or degree in Medical Case Management or related healthcare field.
  • Preferred: Three (3) or more years of clinical, patient care, or case management experience.
  • Active, unrestricted licensure in the State of Michigan (if applicable).
  • CCM or ACM certification preferred.

Skills & Competencies:
  • Computer literacy required - proficient in Word, Excel, Office 365, and a fast learner with case management software.
  • Comfortable using both iPad and HP or Mac computers.
  • Excellent written and verbal communication skills.
  • Strong interpersonal and advocacy abilities.
  • Ability to work independently in the field and maintain timely documentation.
  • Reliable, professional, and detail-oriented.
  • Preferred Languages: Arabic, Spanish, Tagalog, Hindi, or other language (bilingual candidates strongly encouraged to apply).

Transportation Requirements:
  • Must have reliable, insured transportation for travel throughout Michigan.
  • Must maintain a valid driver's license and insurance coverage in Michigan.

Work Schedule & Pay
  • Full-Time: Monday through Friday, 8:00 a.m. - 5:00 p.m. EST
  • Part-Time: Flexible scheduling available.
  • Compensation:
  • $22-35/hour depending on experience, licenses, certifications and degree
