We’re hiring a Home Health & Hospice RN Case Manager in Houston, Texas who wants autonomy in the field, a stable schedule, and the opportunity to provide compassionate care to patients in their homes.
You’ll step into a clinical role focused on skilled nursing visits, patient assessments, care coordination, medication management, and ongoing communication with physicians, patients, and families. You’ll work closely with an interdisciplinary team to ensure quality outcomes and continuity of care across both home health and hospice services.

What makes you a great fit
• You hold a clear and active Texas RN license
• You have Home Health and/or Hospice experience
• You have Medicare documentation experience
• You possess strong assessment, case management, and organizational skills
• You’re comfortable independently managing a patient caseload
• You have excellent communication and patient care skills
Compensation & Benefits
• Salary: $80,000–$85,000
• Health insurance with $250 reimbursement per pay period
