Home Health & Hospice RN Case Manager
Houston, Texas

We’re hiring a Home Health & Hospice RN Case Manager in Houston, Texas who wants autonomy in the field, a stable schedule, and the opportunity to provide compassionate care to patients in their homes.

You’ll step into a clinical role focused on skilled nursing visits, patient assessments, care coordination, medication management, and ongoing communication with physicians, patients, and families. You’ll work closely with an interdisciplinary team to ensure quality outcomes and continuity of care across both home health and hospice services.
