The Discharge Planner works under the director of the RN Case Manager and/or /Social Worker to complete referrals for post-acute services for patients and assists with tasks that do not require a clinical license or degree.

  • Arranges post-discharge services, creates and follows-up on electronic referrals using the Tenet Case Management system,
  • Reviews patient choice letters with patients/families for required signatures,
  • Provides follow up Important Message to Medicare patients prior to discharge,
  • Communicates with patient, families and other members of the care team, completes tasks assigned by Case Manager and Social Work staff,
  • Make copies, send faxes, and completes phone calls,
  • Completes process reviews or audits as requested.
  • Completes established competencies for the position within designated introductory period.
