Job Description
The Care Manager RN will provide inpatient care management services, focusing on effective patient care coordination and discharge planning. The position involves floating between units based on patient care needs, utilizing a strong background in care coordination and case management. The role requires adaptability in scheduling to accommodate staff needs, including vacation and PTO utilization.
Responsibilities
  • Coordinate care for patients across various units, ensuring smooth transitions and continuity of care.
  • Perform discharge planning and continue stay reviews following CMS and HEDIS guidelines.
  • Assess patient needs and collaborate effectively with interdisciplinary teams.
  • Ensure compliance with HIPAA and Joint Commission standards.
  • Participate in quality improvement initiatives related to patient care management.
Required Experience / Certifications / Licensure
  • Minimum of 3 years of inpatient care management experience required.
  • RN license is mandatory.
  • BLS certification is required.
  • Case management certification preferred.
  • Experience using Epic charting system is required.
