One of our clients is seeking a Supportive Housing Case Manager to provide service in the Bloomfield, CT area.
CLIENT'S AVAILABLE HOURS
These are full-schedule client opportunities and the hours are Mon-Fri 8:30 - 4:30.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Identify the needs of the clients and establish social services support to assist them as they receive their new housing.
* Identify eligibility for housing, assist with application for housing voucher.
* Assist with finding and securing apartments for clients
* Work with the homeless population to assist in screening their documents including sensitive information.
* Assist with identification and documentation for securing basic needs and entitlements such as Medicaid, housing subsidies, food subsidies, etc.
* Initiate referrals for mental health and addiction treatment services as needed.
* Encourage social supports such as peer activities and groups and developing community connections.
* Assist with employment support including resume writing, job searching, and interviewing skills as appropriate.
* Continue to provide Case Management activities for the newly housed clients even after they receive their housing in order to assist them in their continued success.
CLIENT'S REQUIRED SKILLS AND EXPERIENCE
* Bachelor's Degree strongly preferred however strong housing case management experience is also considered
* Case Management experience in Supportive / Transitional Housing is required
