Job description

Shift: 8:00 AM - 4:30 PM | 3 days/week (+ every 4th weekend)

Provide compassionate care and make a meaningful difference in patients’ lives as a Home Health/Hospice Aide. In this role, you’ll deliver high-quality personal care, homemaker services, and companionship to patients in their homes while supporting the RN Case Manager to ensure coordinated, patient-centered care.

  • Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility  
  • Provide light housekeeping, meal preparation, and companionship  
  • Support medication administration and simple procedures under RN guidance  
  • Observe and report changes in patient condition or home environment to the RN Case Manager  
  • Maintain accurate documentation and ensure compliance with care plans and regulatory standards  
  • Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives  
  • Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundaries 

Education:  

  • Completion of a 120-hour Nursing Assistant course.  
  • High School Diploma/GED High School diploma or GED 

License(s)/Certification(s):  

  • Valid Driver’s License (IA, IL, or WI) – Required upon hire  
  • Basic Life Support (BLS) Certification – Maintain current certification through the American Heart Association (Required upon hire)  
  • Certified Nursing Assistant (CNA) – State of Iowa (IADIA) or Illinois (IDPH) preferred  
  • Child Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire  
  • Adult Dependent Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire

Experience:   

  • Completing of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care or Hospice Aide/CNA work after completion
