What is CarepathRx?

CarepathRx is a healthcare company that specializes in pharmacy services and medication management solutions for health systems, hospitals, and patients. The company works with hospitals and health systems to optimize specialty and home infusion pharmacy operations, improve patient outcomes, and lower costs. CarepathRx offers a comprehensive suite of services, including specialty pharmacy, home infusion, and medication therapy management, all designed to streamline the continuum of care for patients transitioning from hospital to home. Their services help healthcare providers enhance patient care while managing complex medication regimens.

What are the typical collaboration points for a pharmacist working at CarepathRx with other healthcare professionals?
