By advocating for patients and ensuring all parties are informed, they help streamline care plans, improve patient outcomes, and reduce gaps in service. This role requires strong interpersonal and organizational skills, as well as the ability to manage multiple cases effectively.

What is a Care Connection?

Care Connection staff may include nurses, social workers, or care coordinators who act as liaisons throughout the healthcare process.
