Total Care Connect (TCC) is a mobile integrated health organization delivering in-home clinical and preventive care to members across Ohio and surrounding regions. We support health plans, health systems, and value-based organizations by reaching members where they are โ€” in their homes and communities โ€” to improve access, close care gaps, and reduce avoidable utilization.ย 

As a tech-enabled, field-based care delivery organization, our teams provide a range of services including preventive care, chronic condition support, transition-of-care visits, member engagement, and navigation. We operate with a focus on high-quality member experience, operational excellence, and coordinated care across clinical, administrative, and remote teams.ย

Compensation & Benefitsย 
  • Hourly pay: $28 - 30/hr based on experience

Positionย Summaryย 

Theย Communityย Paramedicย deliversย high-qualityย in-homeย clinicalย careย asย partย ofย Totalย Careย Connectโ€™sย Mobile Integrated Healthcare programs. This role focuses on proactive patient care for high-risk populations, supporting hospital readmission reduction, emergency department avoidance, and improved chronic disease management. Community Paramedics conduct scheduled patient visits, perform clinical assessments, deliver care within established protocols, and collaborate with virtual providers and interdisciplinary care teams to provide coordinated, patient-centered care.ย 

Community-Based Clinicalย Careย 

  • Conduct scheduled patient visits within community-based careย programs.ย 

  • Performย comprehensiveย patientย assessmentsย includingย vitalย signs,ย symptomย evaluation,ย medication reconciliation, and environmental safety review.ย 

  • Deliver care within established clinical protocols and standingย orders.ย 

  • Provideย treatmentsย suchย asย woundย care,ย medicationย administration,ย andย point-of-careย testingย whenย applicable.ย 

  • Identifyย earlyย signsย ofย clinicalย deteriorationย andย escalateย concernsย throughย clinicalย orย operationalย pathways.ย 

Care Coordination & Patientย Supportย 

  • Support continuity of care following hospital discharge or acute medicalย events.ย 

  • Assistย patients in understanding care plans and follow-upย appointments.ย 

  • Identify barriers toย recoveryย including environmental or social determinants ofย health.ย 

  • Collaborateย withย careย coordinationย teamsย toย supportย patientย stabilizationย andย long-termย healthย management.ย 

Documentation &ย Communicationย 

  • Completeย accurateย andย timelyย documentation within the electronic patient care recordย system.ย 

  • Ensure documentation supports clinical accuracy, medical necessity, and payerย requirements.ย 

  • Maintain communication with operational teamsย regardingย visit updates and patientย needs.ย 

  • Participate in documentation reviews and quality improvementย initiatives.ย 

Requiredย Qualificationsย 

  • Current Ohio Paramedic certification in goodย standingย 

  • Minimum 2โ€“3 years of paramedic fieldย experienceย 

  • Valid driverโ€™s license with acceptable drivingย recordย 

  • Strong clinical assessment and patient educationย skillsย 

  • Experience using electronic documentationย systemsย 

Workย Environmentย 

  • Field-based role providing care in patient homes, partner offices, or community clinicalย settings.ย 

  • Travelย betweenย patientย homes,ย partnerย offices,ย andย operationalย locationsย withinย theย assignedย region

  • Exposureย toย variedย homeย environmentsย includingย pets,ย limitedย lighting,ย clutter,ย stairs,ย and environmental hazards.ย
