Location:ย Grand Junction, CO
Schedule:ย Monday โ€“ Friday, 8:00 AM โ€“ 4:30 PM

Position Overview

Community Hospital is seeking an experienced and passionateย Cardiopulmonary Rehab Supervisorย to lead our outpatient cardiac and pulmonary rehabilitation programs. This is a unique opportunity to make a meaningful impact by helping patients improve their health and quality of life through supervised exercise, education, and support.

This role is ideal for aย Registered Nurse (RN)ย orย Registered Respiratory Therapist (RT)ย with strong leadership skills, a background in rehab services, and a commitment to delivering exceptional, patient-centered care.

Key Responsibilities

Program Leadership & Development

  • Oversee day-to-day operations of the Cardiopulmonary Rehab program

  • Develop and implement evidence-based rehab protocols and education

  • Ensure compliance with regulatory standards and internal policies

  • Collaborate with the Director of Respiratory Therapy and other departments to improve service delivery

  • Market and promote the program to providers and the community

Staff Supervision & Support

  • Supervise and schedule program staff

  • Provide coaching, training, and performance feedback

  • Ensure annual competencies and ongoing professional development

  • Foster a positive, team-oriented environment focused on patient outcomes

Patient Care & Clinical Oversight

  • Perform assessments and develop personalized rehab care plans

  • Monitor patient progress during exercise and recovery

  • Provide education and support on disease management and lifestyle changes

  • Review and document patient progress, coordinate follow-up care

  • Assist with insurance verification and scheduling as needed

Community & Interdepartmental Engagement

  • Participate in community health events (e.g., health fairs, wellness programs)

  • Represent the program on committees and workgroups

  • Communicate updates and share insights across departments

Qualifications

Required:

  • Active Coloradoย Registered Nurse (RN)ย orย Registered Respiratory Therapist (RT)ย license, or ability to obtain prior to start
  • Bachelorโ€™s degree in Nursing, Exercise Science, Exercise Physiology, Kinesiology, or a related field
  • Minimum of one year of experience in a cardiopulmonary rehabilitation setting
  • Preferred: Three to five years of experience in a clinical leadership role
  • Certified Clinical Exercise Physiologist (CEP)ย certification, or willingness to obtain upon hire
    Compensation & Benefits
    • Hourly Rate:ย $43.00 โ€“ $49.45, based on experience and education
