This role is entirely remote, allowing you to engage with clients from the comfort of your own home while maintaining a balanced professional and personal life. Our online platform enables seamless case management, patient interactions, and administrative functions, all compliant with HIPAA standards. • Comprehensive Assessments: Conduct thorough psychosocial evaluations for a variety of mental health disorders, ensuring that all aspects of patient history and presenting issues are considered in the treatment plan.
• Individualized Care Plans: Develop and modify treatment plans to address changes in patient conditions, employing evidence-based interventions to achieve optimal results.
• Crisis Management: Assess and respond to crisis situations with immediate and appropriate resources, including referrals to in-need community services and support.
• Multidisciplinary Collaboration: Work closely with patients, their families, and healthcare providers to ensure an integrated approach to care, facilitating communication and coordination among involved parties.
• Discharge Planning: Collaborate with the care team to develop effective discharge plans that ensure continuity of care through appropriate referrals and follow-ups.
• Patient Advocacy: Serve as an advocate for patients and their families, ensuring their needs are effectively communicated and met within the healthcare system.
• Cultural Competency: Provide culturally sensitive care, recognizing and respecting the diverse backgrounds and experiences of each patient.

You will work with a varied patient demographic, addressing a spectrum of mental health issues such as anxiety, depression, trauma-related disorders, and emotional disturbances. • Master's degree in Social Work (MSW) or a related field from an accredited institution.
• Current state licensure as an LCSW, LMFT, LPC, or Psychologist.
• Strong familiarity with community mental health resources and an ability to navigate these systems.
• Exceptional written and verbal communication skills for effective patient interaction and documentation.
• Ability to build rapport and establish trusting relationships with clients to promote healing.

• Experience providing therapy in a community health clinic setting.
• Proficiency with electronic health record (EHR) systems and other digital therapeutic tools.
• Diverse background in dealing with various mental, emotional, and behavioral difficulties.

• Annual salary ranging from $90,000 to $100,000, commensurate with experience.
• Potential sign-on bonus of up to $1,000.
