About the Role
We are seeking dedicated and clinically sound RNs and LPNs to provide skilled nursing care to patients in their homes. This role includes intermittent visits, private duty shifts, and care plan coordination. Key Responsibilities
Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments Carry out shift-based care for patients needing extended clinical support Monitor and document patient condition and progress Collaborate with physicians and interdisciplinary teams to update care plans Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation required for RN) Uphold infection control standards and agency protocols.

Qualifications
