Your Specific Responsibilities:
The Health Services Representative (HSR) is a full-time position within the Office of Community Health and Planning (OCHP) at the Florida Department of Health (FDOH) in Miami-Dade County. The incumbent serves as a member of the Community Health Action Team (CHAT), a mobile Public Health Response Outreach team in the OCHP and is responsible for the provision of specialized public health direct services and outreach within the FDOH, the community, at worksites and schools in Miami-Dade County. This includes professional work planning, implementing and conducting health promotion and public health education programs for clinics, schools, worksites and community groups. They report directly to the Senior Community Health Nursing Supervisor.
Promotes health and chronic disease control through prevention, education, advocacy, regulation, and service. Supports health promotion activities such as biometric screenings and education classes provided to patient/client individuals and groups. This is work related to detecting, controlling, and preventing obesity and other chronic diseases (such as high blood pressure, high cholesterol, diabetes, and cardiovascular disease), and communicable diseases such as sexually transmitted diseases as the need arises in the community. Participates in worksite wellness initiatives within the FDOH, as well as other businesses and organizations, in Miami-Dade.
Make appropriate referrals to medical care for clients as indicated and completes the necessary follow-up. Maintains strict confidentiality with clients and records to ensure compliance with HIPPA rules, regulations, and client privacy. Provides appropriate community resources and services based on client needs. Attends and supports health fairs, health promotion events and community presentations. Plans, organizes, and coordinates their own work assignments, collaborating with other members of CHAT and OCHP staff (including other Senior Community Health Nurses, Health Services Representatives, Community Health Workers, and Health Educators) to ensure the essential public health needs of the community are met. Assists the other senior team members in creating culturally sensitive health education and disease control program literature, posters, flyers, and messages. Assists with the planning and implementation of new clinical and outreach programs as needed.
Assists with establishing and maintaining community contacts, scheduling, and/or packing of presentation materials, promotional items, and educational materials. Assists with setting up and dismantling of all supplies and equipment at health fairs and events. Maintains appropriate records and documentation of services provided, hours worked, and official business mileage traveled. Assists with the preparation of monthly/quarterly reports to monitor strategic objectives and health outcomes for the OCHP. Assists with maintaining the inventory of health education literature, promotional items, and biometric screening equipment. Attends trainings and in-services for personal and professional growth. Assists with the training and orientation of other CHAT members as needed.
Assists with creating the monthly Worksite Wellness Newsletter for DOH Miami-Dade employees highlighting the health awareness topics and/or observances for the month.
Supports the activities and initiatives of the Consortium for a Healthier Miami-Dade, a collaborative effort between the FDOH in Miami-Dade County and various community partners with a vision of a healthy environment, healthy lifestyles and a healthy community for all Miami-Dade County residents and visitors. Represents the Consortium for a Healthier Miami-Dade at health fairs and community events.
