  • Collaborate with primary care teams to identify and manage psychosocial issues and facilitate linkages with various treatment providers as necessary.
  • Provide evidence-based, focused interventions for patients seeking mental health support, including crisis management strategies when required.
  • Monitor progress of patients, initiating follow-ups to analyze treatment efficacy and adapt plans according to changing needs.
  • Document therapeutic progress and maintain detailed clinical records while adhering to ethical standards and confidentiality regulations.
  • Educate patients on self-management techniques and coping strategies to empower them in their journey toward recovery.
  • Work collaboratively with clinic management and team members to enhance behavioral health services and evaluate program effectiveness.
  • Cultivate strong, therapeutic relationships with patients, ensuring they feel understood and supported throughout their treatment process.
  • Participate actively in team meetings and case discussions to pitch in insights on best practices and improve overall service delivery.
