POSITION SUMMARY
The Medical Case Manager provides comprehensive case management, care coordination, skills training, and client advocacy services to individuals receiving medical and behavioral healthcare services. Working collaboratively with clients, families, healthcare providers, and community partners, the Medical Case Manager assists clients in accessing medical treatment, navigating healthcare systems, improving health outcomes, and achieving goals identified in their individualized service plans. The Medical Case Manager is an active member of the interdisciplinary treatment team and promotes client recovery through trauma-informed, person-centered care.
Essential Duties and Responsibilities
  • Lives out the INcompass Healthcare promise by consistently displaying compassion, treating all individuals with dignity, and providing exceptional care for everyone.
  • Incorporates the six principles of trauma-informed care throughout all aspects of service delivery.
  • Conducts assessments and ongoing reassessments to identify client medical, behavioral health, social, and support service needs.
  • Develops, implements, and monitors individualized service plans in collaboration with clients, families, and treatment team members.
  • Coordinates healthcare services, including medical appointments, specialty care referrals, medication management support, and follow-up care.
  • Provides direct case management services in accordance with agency policies, procedures, and regulatory requirements.
  • Assists clients in accessing healthcare benefits, insurance coverage, transportation, community resources, and social support services.
  • Provides crisis intervention and crisis management services as needed.
  • Teaches and models health management, self-advocacy, wellness, medication adherence, and independent living skills identified in the service plan.
  • Advocates for clients to ensure access to appropriate medical, behavioral health, and community-based services.
  • Collaborates with physicians, nurses, therapists, social service agencies, schools, and other community partners to coordinate care.
  • Maintains regular communication with internal and external providers to support continuity of care.
  • Monitors client progress toward treatment goals and adjusts interventions as appropriate.
  • Other duties as assigned.

Minimum Qualifications (Knowledge, Skills, and Abilities)
  • CNA or MA; or equivalent combination of education, professional experience, and lived experience.
  • Experience working with individuals with medical, behavioral health, or social service needs.
  • Effective communication and client engagement skills
