Where You Will Work: This position is based in Lantern’s Supportive Housing program in Manhattan, serving formerly homeless families living with chronic physical illness and/or mental health challenges.


About the Role: We are seeking an experienced Bilingual Case Manager to support formerly homeless families in stabilizing their lives, maintaining housing stability, and achieving their physical, emotional, vocational, and social goals. The Bilingual Case Manager will assess client needs, develop service plans, connect residents to appropriate services, monitor progress, and maintain thorough documentation. The ideal candidate is passionate about helping people, demonstrates strong clinical judgment, and can work both independently and as part of an interdisciplinary team. This individual will have experience supporting individuals impacted by mental health challenges, substance use, legal involvement, and chronic medical conditions.


Essential Functions:

  • Provide integrated counseling and case management services that promote health, wellness, personal growth, and housing stability.
  • Deliver services including benefits and entitlement assistance, case management, group programming, individual counseling, advocacy, referrals, and resource coordination.
  • Develop client-centered, recovery-oriented service plans identifying short- and long-term goals.
  • Utilize a harm reduction approach when addressing mental health and substance use concerns.
  • Conduct regular apartment visits, community outreach, and escorts to appointments as needed.
  • Develop safety plans and collaborate with the team to de-escalate and manage crises.
  • Maintain accurate and timely documentation, including progress notes, case contacts, and required reports for regulatory and funding agencies.
  • Participate in staff meetings, clinical meetings, and supervision. Serve as liaison with healthcare providers, mental health providers, substance use programs, employment services, and care coordination agencies.
  • Collaborate with Leasing and Compliance teams to support ongoing housing stability.
  • Participate in ongoing professional development and evidence-based training programs.
  • Perform other related duties as assigned.


Required Qualifications:

  • High School Diploma, HSE, or GED required.
  • Minimum of two (2) years of social services experience required.
  • Valid Food Handler’s License or ability to obtain within six (6) months of hire required.
  • Experience working with homeless, mentally ill, substance use, justice-involved, and/or low-income populations.
  • Strong organizational skills and ability to manage a diverse caseload. Ability to work independently and as part of an interdisciplinary team.
  • Ability to work effectively under pressure, including during crisis situations.
  • Experience with electronic case management systems.
  • Bilingual proficiency in English and Spanish required.


Work Environment: This position operates in a supportive housing site in Manhattan and works with individuals who may experience mental illness, substance use, disability, HIV/AIDS, and/or other chronic health conditions.


Physical Demands: While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee may lift and/or move up to 10 pounds.
