Behavioral Health Consultant

Prism Health is the only nonprofit LGBTQIA+ health center providing comprehensive primary care, behavioral healthcare, and pharmacy services in the Pacific Northwest (Oregon, Washington, and Idaho). Since opening its doors in 2017, Prism Health has become a model for culturally specific care and now serves over 4,000+ patients with a wide range of services, from day-to-day primary care and pharmacy needs to targeted services like gender-affirming care and specialized population health screenings.

Prism Health a program apart of Cascade AIDS Project is excited to announce that we are hiring a skilled and team-oriented Behavioral Health Consultant. The Behavioral Health Consultant (BHC) provides brief, evidence-based behavioral health services to adult patients as an integrated member of the primary care team at Prism Health. Operating within a Primary Care Behavioral Health (PCBH) model, the BHC delivers time-limited interventions addressing behavioral, emotional, and psychosocial factors impacting health and functioning. The role also involves warm same-day handoffs and real-time consultation with primary care team providers within a population-based, team-driven model.

This is a Full-Time role working at our Prism Clinics located on SE Belmont and N Morris. The schedule for this role is Monday-Friday 8:30am-5:00pm with some flexibility needed for meetings. This position is fully on-site.

Behavioral Health Consultant III – Licensed with master's degree in mental health-related field (Counseling, Psychology, or Social Work) Salary is $93,151.66

Behavioral Health Consultant IV pay –Licensed with a Doctoral degree in mental health-related field (Counseling, Psychology, or Social Work) Salary is $95,476.68

This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Who You Are

  • Master's or doctoral degree in mental health-related field (Counseling, Psychology, or Social Work)
  • BHC III and BHC IV - Active Oregon license in good standing (LCSW, LPC, LMFT, PsyD, or PhD)
  • Valid driver's license and reliable transportation for travel between sites
  • Experience working in an integrated primary care or medical setting
  • Experience providing behavioral health consultations or brief interventions
  • Proficiency in evidence-based therapeutic modalities (ACT, CBT, DBT, motivational interviewing, harm reduction)
  • Demonstrated ability to provide crisis intervention, including conducting risk assessments and developing safety plans for patients in distress, while coordinating with the care team and community resources as clinically indicated

What You'll Do

  • Provide brief behavioral health consultation and intervention through same-day warm handoffs and scheduled follow-up visits
  • Conduct targeted assessments and develop brief, problem-focused treatment plans to support patients in primary care, including helping patients identify personal values and develop realistic, behavior-based strategies for improved health and functioning
  • Provide urgent behavioral health support, including suicide risk assessment and safety planning as clinically indicated
  • Deliver patient education through individual consultations and group-based interventions to enhance treatment engagement and facilitate health behavior change
  • Provide behavioral health support for patients managing chronic medical conditions, including support for treatment adherence and health-related behavior change
  • Coordinate referrals to specialty or long-term behavioral health providers when clinically indicated, collaborating with case management and care coordination staff as appropriate
