Case Manager- Full Time | Raleigh Oaks Behavioral Health | Garner, North Carolina
About the Job:
Schedule: Monday- Friday 8am-4:30pm
The Case Manager supports the clinical services department by providing case management to the different units within the facility. Responsible for the overall discharge resource connection and aftercare planning needs and placement of the patient based on their individual needs and as directed by the treatment team. Maintains routine contact with referral sources and providers while maintaining resource file. Participates in quality care monitoring for the department.
Roles and Responsibilities:
ESSENTIAL FUNCTIONS:
  • Participates in the development of the service plan.
  • Implements appropriate activities and/or programs. Implements services to patients in accordance with their individualized service plan (ISP) and within the case manager's scope of practice.
  • Collaborates with the patient and patient's family or significant others to implement an effective aftercare service plan, explaining the available options to the team, including the advantages and disadvantages of each option.
  • Maintains the patient's comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning). Coordinates releases and faxing records to providers.
  • Coordinates referrals and connections for patient with mental illness and/or substance use including accessing psychiatric services, therapy services, community-based services, employment, housing or other placement options.
  • Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patients, their family, behavioral and general medical health care providers, community resources and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented.
  • Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources.
  • Coordinates referral or completes the screening and assessment on all patients on caseload for financial entitlements (AHCCCS, SSI/SSD etc.); completes AHCCCS applications on all patients on caseload meeting criteria.
  • Assures that discharge planning begins at admission. Obtains collateral input from providers and supports.
  • Ensures the development and implementation of transition, discharge, and aftercare plans prior to discontinuation of behavioral health services.
  • Ensures all contact with referral source and/or other treatment providers is done within designated time frame and documented accordingly.
  • Makes arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
  • Employs appropriate crisis intervention techniques as needed.
  • Communicates appropriate patient information to the treatment team; participates in team discussion and evaluation of patient goals, objectives and motivations.
  • Schedules after care appointments for patients with PCPs, psychiatrists, next level therapy programs, living arrangements, etc.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
  • Bachelor's Degree in a behavioral health area, marketing, business administration or related field preferred.
  • One or more years previous related experience in Behavioral Health preferred.
  • Previous experience in case management preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:
  • CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
  • First aid may be required based on state or facility requirements.

WORK LOCATION:
This position is onsite at the facility and is not a remote position.
SUPERVISORY REQUIREMENTS:
This position is an Individual Contributor.
