40 hours per week - Tuesday to Saturday 9:00AM to 5:00PM
Position Summary
The Licensed Practical Nurse (LPN) provides direct nursing care and clinical oversight to individuals residing in a group home setting. The LPN supports individuals with developmental and/or physical disabilities by ensuring safe medication administration, monitoring health conditions, and collaborating with interdisciplinary teams to promote optimal health and quality of life.
This role requires strong clinical judgment, effective communication, and the ability to support and educate direct care staff in accordance with DDS and Medication Administration Program (MAP) guidelines.
Key Responsibilities
Clinical Care and Monitoring
  • Provide direct nursing care in accordance with individualized care plans
  • Monitor and assess residents' health status; identify changes and report concerns promptly
  • Perform routine nursing assessments (skin integrity, vital signs, pain, etc.)
  • Assist with medical appointments and follow-up care coordination

Medication Administration and MAP Compliance
  • Administer medications and treatments per physician orders and MAP regulations
  • Ensure proper medication storage, documentation, and chain of custody
  • Conduct medication counts, especially for controlled substances
  • Report and document medication occurrences per policy

Care Coordination
  • Communicate with physicians, pharmacies, VNA, therapists (PT/OT/Speech), and families
  • Assist in implementing and updating care plans
  • Coordinate lab work, diagnostic testing, and follow-up appointments

Staff Support and Training
  • Provide guidance and oversight to direct support staff on health-related needs
  • Reinforce proper techniques (e.g., feeding protocols, positioning, use of adaptive equipment)
  • Participate in staff training related to health and safety procedures

Documentation and Compliance
  • Maintain accurate and timely documentation in accordance with agency, DDS, and DPH standards
  • Complete incident reports and nursing notes as required
  • Ensure compliance with infection control and safety protocols

Health and Safety
  • Monitor for signs of illness, injury, or decline in condition
  • Respond to medical emergencies and initiate appropriate interventions
  • Promote preventative care (hydration, nutrition, skin care, mobility support)

Required Qualifications
  • Active LPN license in the state of Massachusetts
  • Current CPR/BLS certification
  • Strong assessment, communication, and organizational skills

Preferred Qualifications
  • Experience in a group home or community-based setting
  • Experience with individuals with developmental disabilities (preferred)
  • Knowledge of DDS regulations and MAP policies (strongly preferred)
  • Experience with electronic medication administration systems (e.g., Impruvon)

Physical Requirements
  • Ability to assist with transfers and mobility (may include Hoyer lifts)
  • Ability to stand, walk, bend, and lift as needed
  • Ability to respond quickly in emergency situations

Work Environment
  • Residential/group home setting
  • May include evenings, weekends, and holidays depending on schedule

Salary: $36.00 to $39.00 per hour to commensurate with education and experience.
