Qualifications:
  • Be a high school graduate or have a certificate of successful completion of the General Educational Development Test.
  • Have three (2) years of responsible full time clerical experience.
  • Ability to make general decisions in accordance with departmental policies and procedures.

Duties and Responsibilities:
  • Demonstrate ability to engage individuals served in a welcoming, hopeful, empathetic manner regardless of disability or phase of recovery.
  • Collaborate with primary care physicians, mental health specialists, and other healthcare professionals to ensure that individuals receive comprehensive integrated care for both physical and mental health.
  • Monitor initial and ongoing health screenings, including but not limited to medical history, behavioral health issues, and any co-occurring disorders.
  • Work with multidisciplinary teams to support the development of personalized, holistic person-centered treatment plans that address both mental health disorders (e.g., depression, anxiety, substance use) and physical health conditions (e.g., diabetes, hypertension).
  • Gather materials and/or resources to share with care teams.
  • Monitor individuals progress and facilitating regular follow-up appointments to ensure continuity of care. This includes providing referrals and resources to specialty services and/or MyCare, when needed.
