BayCare Mental and Behavioral Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in Astoria, New York. This full-time remote position is an integral part of our Hospice Center, where your expertise will enhance the emotional and social well-being of our patients during critical stages of their rehabilitation journey. The role offers a salary range of USD 75,000 to 100,000 per year, along with a sign-on bonus for qualified candidates.

Regular business hours are expected, but flexibility for occasional evening and weekend shifts will be required based on the needs of our patients and assigned caseloads.

• Conduct in-depth psychosocial assessments by interviewing patients, observing interactions, and reviewing relevant medical records to identify emotional, social, and environmental factors that may influence recovery.
• Provide compassionate counseling and support to patients and their families, helping them navigate adjustment issues and emotional challenges related to health changes.
• Collaborate closely with the multidisciplinary rehabilitation team to formulate individualized treatment plans that prioritize psychosocial needs alongside medical ones.
• Assist patients in connecting with essential community resources, such as support groups, vocational rehabilitation services, and housing assistance to facilitate their recovery.
• Educate patients and families about available community resources and post-rehabilitation services, ensuring they are well-prepared for life after care.
• Engage actively in discharge planning processes, ensuring smooth transitions for patients returning home or to other care environments, prioritizing their ongoing support and well-being.

Our Hospice Center serves a diverse patient population dealing with various conditions ranging from chronic illnesses to those recovering from significant medical events. You will play a critical role in understanding the unique emotional and social challenges these individuals face, promoting their resilience, independence, and successful reintegration into their communities.

• A Master’s degree in Social Work from an accredited program is mandatory.
• Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is required.
• Demonstrated knowledge of psychological interventions and counseling techniques specific to rehabilitation settings.
• Strong communication and interpersonal skills are essential to effectively support and engage with patients and their families.

• Experience within a rehabilitation or medical setting is highly preferred, giving you the capabilities to navigate complex patient interactions and multidisciplinary teamwork.
• A strong understanding of adjustment issues, grief, and coping mechanisms as they relate to patients faced with significant life changes.
