Freedom at Home Dialysis, Expedient Home Health Care and Green Hills Hospice are sister agencies. We are a committed team of Healthcare professionals dedicated to the ethical pursuit of improving the quality of life of all our patients and presenting ourselves as advocates and leaders in our profession.
Job Description
  • Provides direct patient services that are physician ordered.
  • Provides hands-on personal care and support services under the direction and supervision of a RN.
  • Provides assistance to patients with ambulation and safe transfers per agency policies.
  • Reports and condition changes to Case Manager and documents any services provided and changes noted in patients condition.
  • Participates in care coordination meetings per company policy.
  • Must be able to lift up to 50 lbs and assist with transfers of patients.
  • Must be able to drive around Metroplex area to patients residence to provide care.
  • Must respect patients home and environment.
  • Must keep up required continuing education requirements for a CNA.

Qualifications
  • Current CNA license
  • Current Drivers License
  • Must have 1 year experience in Home Health, nursing home or hospital setting.
  • Must be computer literate and be able to document electronically.
  • Must have current CPR certificate
  • Must be able to pass State required onboarding testing.
  • Must have reliable transportation and current auto insurance
