Our Home Health RN provides and directs provisions of nursing care to patients in their homes as prescribed by the physician. The RN coordinates the patient's individualized plan of care with other health care professionals involved and helps to achieve and maintain continuity of patient care.
Qualifications Required:
  • Graduate of an Accredited School of Nursing
  • Registered Nurse License
  • Valid CPR certification
  • Valid Florida Driver's License and have access to a personal vehicle.

Qualifications Preferred:
  • OASIS Experience
  • BSN

Duties and responsibilities include:
  • Responsible for the coordination and delivery of quality of care as prescribed by physician's order.
  • Develops initial care plan, provides subsequent visits as directed by the plan of care.
  • Documents all assessments, activities, and care coordination in the Wellsky Kinnser EHR system.
  • Observes and reports symptoms, reaction to treatments, drugs, and changes in the patient's physical or emotional condition.
  • Perform on-going periodic system assessment and data collection in a systematic manner, focusing on: psychological, psychosocial, cultural spiritual and cognitive status; nutrition; pain; patient/family education; family involvement and patient advocacy; patient and patient's equipment needs.
  • Facilitates timely discharge based on goals and in accordance with client policies and procedures.
  • Educates and instructs the patient, patient's family and other staff as required.
