Job description

As a Home Health RN, you will provide skilled nursing care, complete comprehensive assessments, coordinate individualized care plans, and collaborate closely with physicians, caregivers, and interdisciplinary teams to support patients through recovery, chronic disease management, and overall wellness. Your clinical expertise and compassionate approach will empower patients to maintain independence and achieve the highest possible quality of life in the comfort of their homes.

Responsibilities

As a Home Health RN, you will:

  • Perform initial and ongoing comprehensive assessments of patientsโ€™ physical, functional, psychosocial, and environmental needs
  • Develop, implement, and update individualized plans of care to ensure quality and continuity of patient care
  • Determine patient eligibility and appropriate levels of care for admission and recertification in accordance with home health guidelines
  • Provide skilled nursing interventions and initiate preventative and rehabilitative nursing procedures
  • Educate patients and caregivers on disease management, medications, nutrition, safety, and self-care techniques
  • Assess caregiver support systems and identify ongoing patient and family needs throughout the course of care
  • Apply infection control practices and standard precautions during all patient care activities
  • Collaborate with physicians and interdisciplinary team members regarding changes in patient condition and care needs
  • Supervise and provide clinical direction to Home Health Aides and LPN/LVNs to ensure quality patient care
  • Complete accurate and timely clinical documentation, progress notes, and care coordination
  • Monitor assigned patient cases to ensure compliance with physician orders and third-party payor requirements

Qualifications
  • Current and active RN license
  • Valid Driverโ€™s License and current auto insurance
  • Current CPR Certification
  • Minimum of 2 years of Home Health RN experience preferred
