Choice Health at Home is seeking an experienced and compassionate Home Health Field Registered Nurse (RN) to join our growing team. This position follows a Thursdayโ€“Monday schedule, offering consistent weekday and weekend coverage while allowing two consecutive days off each week.

As a Home Health RN, you will provide high-quality, patient-centered care in the comfort of patientsโ€™ homes while helping patients achieve independence, improved outcomes, and enhanced quality of life.

What Youโ€™ll Do
  • Complete initial and ongoing comprehensive patient assessments including physical, functional, psychosocial, and environmental needs
  • Evaluate eligibility and establish appropriate levels of home health care services
  • Develop, implement, and update individualized plans of care
  • Educate patients and families on disease management, self-care techniques, medications, nutrition, and safety
  • Determine frequency and scope of services based on patient acuity and caregiver support
  • Perform preventative and rehabilitative nursing interventions
  • Maintain infection prevention and standard precaution practices
  • Supervise and provide clinical guidance to Home Health Aides and LPN/LVNs
  • Document patient progress and clinical outcomes accurately and timely
  • Communicate changes in patient condition to physicians and interdisciplinary team members
  • Ensure compliance with payer requirements and quality standards
Qualifications
  • Current RN License
  • Valid Driverโ€™s License
  • Current CPR Certification
  • Valid Auto Insurance
  • Minimum 2 years of Home Health RN experience strongly preferred
  • HomeCare HomeBase experience preferred
