Your Opportunity:

Integrated Home Care uses a strengths-based Client Centered Case Management practice framework. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family, you will ensure services are provided and modified on an ongoing basis, based on client's response to interventions, incorporating stewardship into service authorization. You will report to the Care Manager and be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients/families in their own residence or an alternate care setting, as determined by the assessment. Client care in a HC setting uses a collaborative team-based approach while working remotely in a community setting. There is an expectation to be a mobile worker and travel is required within the city of Calgary. You will be responsible to operate a laptop computer and mobile phone and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. E.g. lifting of approx. 30 lbs. in/out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common with this job. Learning resources and training is provided to safely perform work. The successful incumbent will be expected to demonstrate proficiency in the following areas: Personal and professional accountability. Systems and creative thinking, communicating and building relationships. Excellent organizational, time management and interpersonal skills. Intermediate computer skills.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Additional Required Qualifications:

This is an on-site position. At least 2 years of nursing experience. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation within the last 12 months. Must have 1-2 years recent Home Care or Community experience.

Preferred Qualifications:

Bachelor's degree in Nursing. Experience working with acute clients and/or long term clients with a focus on chronic disease management. Recent experience in a community-based setting, Home Care experience preferred. Experience in providing case management services. Canadian Nurses Association (CNA) certification in Gerontology. Experience in Home Parenteral Therapy (HPT), wound, skin and ostomy care, palliative support and client.
