Assured and Associates, Home Health Care

This statement sets forth the basic tasks and duties of the job, however it is not intended to provide a detailed description of all functions that may be required to perform the job satisfactorily.

Job Responsibilities:

  • Provide follow-up skilled nursing visits for home health patients.
  • Report patient care/condition/progress to patient's physician on a continuous basis.
  • Prepare visit/shift reports and updates/summarizes patient records
  • Assist with prescribed medications for patients
  • Take vital signs as prescribed.
  • May record and restock medication inventories.
  • Assure that established infection control practices are maintained.
  • Report all hazardous conditions and equipment to the supervisor.
  • Keep the supervisor informed of supply needs.
  • Maintain the confidentiality of patients’ personal care information.
  • To act at all times in a professional manner to visitors, staff and residents in the community.
  • Attendance on the job as scheduled.
  • Wound Care

Job Requirements:

  • High School Diploma
  • Holds a current license in good standing as a RN or LPN in the state of practice
  • CPR/First Aid
  • TB Test Results
