Job Overview

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the communityโ€”from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizationsโ€”including Minnesota Community Care and Sanneh Foundationโ€”to make it easier for people to access what they need to be well.

Ebenezer is seeking a Registered Nurse (RN) for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This role utilizes the nursing process to deliver individualized resident care. The RN delegates, directs, and monitors nursing care delivery and demonstrates sound clinical skills through problem-solving and decision-making.

This Nursing schedule includes;

  • 48 hours every two weeks ; part time
  • Day shift ; 6:00am-2:30pm
  • Transitional Care Unit TCU
  • Block Schedule:
    • Week 1: Thursday and Friday
    • Week 2: Monday, Tuesday, Saturday, and Sunday
  • Every other weekend and holiday rotation
  • Full Benefits package offered


Responsibilities

  • Monitors personal cares provided to assure consistency with each individual plan of care.
  • Observes and monitors cares given by nursing staff through daily rounds.
  • Assures that NARs are completing assignments/documentation. Redirect NAR if cares are not being done according to assignment.
  • Delegates job duties to the nursing assistants within their scope of practice.
  • Gives feedback to RN Charge Nurse/Nurse Manager regarding LPN, NAR performance and provides necessary documentation upon request.
  • Elevates concerns about staff performance, duty refusals to the Charge Nurse, Nurse Manager or/and Director of Nursing
  • Reports problematic areas to RN Charge, Nurse Manager with suggested solutions.
  • Attends mandatory, annual and any other educational opportunities on and off site.
  • Answers call lights.
  • Answers the telephone.
  • Participates in completing audits as assigned.
  • Assists with appointments, transportation, and patient transfer to the ED.
  • Monitors mealtimes to assure that: a) appropriate staff coverage and assistance is given, and b) proper diets are served in a timely manner.
  • Serves as contact with families and outside health care providers.
  • Active in admission and discharge process.
  • Contacts the physician/family on a timely basis when indicated and documents the outcome.
  • Contacts the physician/family of changes in condition when indicated and documents the outcome.
  • Assures that plan of care, medical records and other documentation are current, complete and accurate.
  • Assesses resident needs upon admission and during the length of stay.
  • Identifies needs of residents according to physical, mental, and psycho-social aspects of care maintaining the highest level of functioning.
  • Evaluates plan of care and revises as appropriate.
  • Assures that changes in condition or acute episodes are reflected in the medical record/plan of care.
  • Provides teaching/training to residents, families and staff regarding meds, treatment, and rehab procedures.
  • Completes UDAโ€™s/assessments as directed.
  • Completes Medicare, Antibiotic time outs, Risk Management and other routine documentation in point click care are directed.

  • Basic Life Support (American Heart Assoc or Red Cross)
  • AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor

Preferred Qualifications

  • 1 year nursing experience
  • less than 1 year TCU LPN/RN preferred six months previous experience
