We are hiring for a full time Nurse Case Manager at Valor House in Salt Lake City, Utah!

The Valor House Nurse Case Manager provides nursing care, medication management, and health services for Veterans residing in transitional housing at Valor House - a 72-bed program located on the Salt Lake City VA Medical Center campus. Working under the administrative supervision of the Valor House Program Manager and the clinical oversight of the Medical Director, the RN is responsible for supervising the medication control system in compliance with the Utah Nurse Practice Act, conducting medical needs assessments, coordinating with outside healthcare providers, and addressing the health needs of a population with complex co-occurring mental health, substance use, and medical conditions.

Education and Experience

  • Current, unrestricted Registered Nurse (RN) licensure in the State of Utah - required
  • CPR / BLS Certification - required
  • Valid Driver's License and Insurability - required
  • A bachelor's degree in nursing (BSN) from an accredited program is preferred. An associate's degree in nursing (ADN) with equivalent clinical experience will be considered.
  • Minimum of one (1) year of clinical nursing experience. Experience in SUD treatment, behavioral health, transitional or supportive housing, or Veterans' services is strongly preferred.
  • Experience with Veterans' health needs - including military trauma, TBI, and service-connected conditions - is an asset.

Duties include but are not limited to:

  • Supervises and manages the medication control system at Valor House in accordance with Sections R156-31b-701 and R156-31b-701a of the Utah Nurse Practice Act Rule, including medication storage, administration, documentation, destruction, and error reporting.
  • Conducts medical needs assessments for Veterans at intake and on an ongoing basis, identifying health concerns and developing care plans in collaboration with the interdisciplinary team.
  • Coordinates with outside healthcare providers - including VA medical providers, primary care physicians, specialists, and emergency departments - to facilitate referrals, appointments, and continuity of care.
  • Provides assistance with at-home medical procedures as needed, within the scope of nursing practice, for Veterans who require nursing support in their residential setting.
  • Provides health education to Veterans on topics including medication management, side effect monitoring, chronic disease self-management, substance use and its health effects, nutrition, and preventive care.
  • Documents all nursing contacts, health assessments, medication events, and significant clinical observations in the Electronic Medical Record in accordance with agency standards.
  • Participates in performance improvement activities related to medication management, health outcomes, infection control, and program quality.
  • Participates in case consultations and interdisciplinary team meetings, contributing a nursing and health perspective to Veterans' individualized service plans.
  • Performs similar and incidental duties as assigned.
