Job Summary

Position title: Care Coordinator

Job Summary: The Care Coordinator is responsible for building and maintaining strong relationships with patients, conducting in-person assessments, and developing individualized care plans in coordination with healthcare providers and caregivers. The ideal candidate will serve as a trusted advocate and central point of contact for patients, helping them navigate the healthcare system and access the resources they need to support their health and well-being.

Responsibilities
  • Build and maintain trusted relationships with patients through ongoing communication and support

  • Conduct face-to-face assessments to evaluate patients' medical, social, and emotional needs

  • Develop personalized care plans in collaboration with providers, caregivers, and the care team

  • Coordinate and connect patients with appropriate community resources to address social determinants of health

  • Promote timely access to care, including preventive services and follow-up appointments

  • Facilitate referrals to primary care and specialty providers as needed

  • Assist with scheduling appointments and arranging transportation services

  • Serve as the main point of contact and advocate for patients, caregivers, and the broader care team

  • Educate patients on available health services and community support programs

  • Maintain accurate and up-to-date documentation in accordance with organizational and regulatory standards

  • Ensure high-quality, patient-centered care that improves outcomes and patient satisfaction

Qualifications
  • Bachelor's degree in healthcare, social work, public health, or a related field preferred

  • Associate's degree accepted with relevant experience in healthcare or social services

  • Minimum 2 years of experience in a healthcare setting
