Starting Pay: $23-25/hour (DOE) + Benefits

White Blossom Care Center is looking for a compassionate and detail-oriented Social Services Assistant to join our dedicated team. If you have experience in a Skilled Nursing Facility (SNF) and are passionate about supporting residents and their families, we encourage you to apply.

Position Summary

In this role, you will support the Social Services Director in delivering high-quality, resident-centered care. You will play a key role in addressing residents' psychosocial needs, coordinating services, and ensuring compliance with all regulatory requirements.

Key Responsibilities

  • Assist the Social Services Director with care planning and service delivery

  • Support residents and families with transitions, emotional needs, and care-related concerns

  • Complete documentation, assessments, and progress notes in compliance with state and federal regulations

  • Collaborate with interdisciplinary team members to ensure comprehensive care
