Job description

Hospice Licensed Practical Nurse
Salary: $31.25-$34.13 BOE
Schedule: Friday/Saturday/Sunday| 8 AM- 8 PM
Location: Hamilton County, Ohio, and surrounding area
Requirements of the Hospice LPN role:
  • Licensed Practical Nursing Degree
  • A minimum of 1 year of Clinical experience/Skilled care
  • Hospice experience preferred
  • Valid Drivers License and Auto insurance Coverage
  • Reliable transportation: Willing to travel

About the Hospice LPN role:
  • Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
  • Provide on-call services after hours, and complete scheduled skilled visits.
  • Assist Registered nurses in carrying out the plan of care.
  • Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders.
  • Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
  • Assist in the reinforcement of patient and family education regarding end-of-life care.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
  • Keep the RNCM informed of changes noted during the process.
  • Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading.
  • Perform other duties as directed
