Key Responsibilities
  • Plan, coordinate, manage, and implement customized support packages tailored for clients and their families to help navigate the end-of-life process.
  • Conduct thorough assessments of service users by interviewing them and evaluating their current mental and emotional health, needs, strengths, and vulnerabilities.
  • Develop individual treatment plans that set measurable goals specific to each clientโ€™s situation and needs.
  • Continuously monitor and evaluate client progress, making adjustments to treatment plans as necessary to ensure effective outcomes.
  • Provide informed guidance and recommendation during counseling sessions, facilitating discussions around coping strategies, emotional support, and community resources.
  • Maintain comprehensive and accurate records of client interactions and progress, ensuring confidentiality and compliance with all regulations.
  • Engage in case conferencing and provide documentation and evidence in court when required, supporting clientsโ€™ needs throughout legal processes.
  • Act as a key worker within a multidisciplinary team, collaborating closely with healthcare providers, nursing staff, and spiritual care teams to deliver integrated care.
  • Refer clients and families to additional community services and support resources to assist in their recovery journey and enhance their support network.
Patient Population / Client Focus

Qualifications
