Benefits: Full Benefits Package, PTO, Paid Holidays, CME Allowance, Malpractice Coverage, DEA Reimbursement, and More

Great healthcare rarely happens because of one appointment.

It happens because someone is connecting the dots.

Someone notices the pattern.

Someone coordinates the plan.

Someone ensures nothing falls through the cracks.

At Twin Cities Physicians, that someone is often the Physician Assistant.

We're seeking a PA-C who wants to play a central role in helping patients navigate complex healthcare needs while receiving care directly in the communities they call home.

Our providers serve assisted living communities, memory care settings, and long-term care environments throughout the Twin Cities Metro. Rather than seeing patients in isolation, you'll work closely with nurses, caregivers, families, therapists, and physicians to create continuity, improve outcomes, and help patients receive the care they need at the right time.

Organizations such as Bluestone Physician Services, Genevive, and Lifespark have helped demonstrate the value of community-based medicine. Twin Cities Physicians continues to expand throughout Minnesota by building strong clinical partnerships and delivering care where patients need it most.

Why Providers Are Drawn To This Opportunity

  • $120,000 to $160,000+ total compensation opportunity through salary and productivity incentives.

  • Serve as a key clinical decision-maker across multiple senior communities.

  • Help coordinate care among physicians, nursing teams, therapists, families, and community partners.

  • Build expertise in geriatrics, chronic disease management, preventative medicine, and post-acute care.

  • Join a provider-led organization serving hundreds of senior communities throughout Minnesota.

  • Receive mileage reimbursement, CME support, malpractice coverage, DEA reimbursement, PTO, and comprehensive benefits.

What You'll Be Doing

  • Provide on-site primary care and rounding services throughout assisted living, memory care, and long-term care communities.

  • Perform assessments, chronic disease management, preventative care, and follow-up visits.

  • Develop and adjust individualized care plans.

  • Coordinate care with physicians, nurses, therapists, caregivers, and family members.

  • Support transitions of care and help reduce avoidable emergency room visits and hospitalizations.

  • Ensure continuity of care across a diverse patient population.

Qualifications

  • Active Minnesota Physician Assistant (PA-C) license in good standing.

  • Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, post-acute care, or related settings preferred.

  • Strong communication skills and confidence working collaboratively with interdisciplinary teams.

  • Valid driver's license and reliable transportation.

The most valuable providers aren't always the ones who see the most patients.

They're often the ones who bring everyone together to achieve the best possible outcome.

If you're looking for a full-time opportunity where your clinical judgment, coordination skills, and leadership can directly influence patient care across an entire community, we'd love to connect.
